PATHWAY HEALTH SELECTED AS TRAINING AND IMPLEMENTATION PARTNER FOR THE CONSONUS CO-PILOT APPLICATION

Pathway Health has been selected by Consonus Healthcare as its implementation, training and ongoing support services partner for its new app, Consonus Co-Pilot. This tool was created by Consonus Healthcare to address a need discovered by its sister company, Marquis Companies, for more accurate data reporting in its 24 skilled nursing facilities (SNFs). Through this partnership, Consonus Healthcare will leverage Pathway Health’s vast network of more than 150 clinicians and technology experts to provide comprehensive implementation and training support.

Consonus Co-Pilot focuses on SNFs’ needs based on the changes resulting from the Affordable Care Act (ACA). The Consonus Co-Pilot platform offers live data for most U.S. SNF operators and provides actionable plans within key pay-for-performance areas, including hospital readmission rates, therapy outcome solutions, customer satisfaction and quality outcomes. Prior to this tool, health systems and managed care payers depended on obsolete CMS claims data for tracking hospital readmissions. “Often this inaccurate data was used as the gauge for our industry’s value in the health care continuum,” stated Anthony Laflen, Director of Data Analytics at Consonus Healthcare. “We needed a solution that provided accurate analytics in real time. Data that reflected all patients, not just CMS Medicare patients. Live data.”
